Connected Bradford links anonymous patient data across different healthcare organizations in Bradford District to understand how to improve care.

By linking data from places like hospitals, GP practices, education, housing and councils, researchers can see the full picture of patient needs and outcomes. Linking data enables quicker improvements in health and care service delivery, improve and enable information sharing between organizations, and help plan better services. For example, linking data on elderly patients can help coordinate services proactively to reduce preventable hospitalizations from issues like falls, keeping older people healthy and independent longer. The goal is continuous improvement in patient care and outcomes across Bradford.
