Learning care homes: Continuous improvement of structured referrals.

Many emergency admissions from care homes are unnecessary, cause distress and place pressure on the NHS and social care. HealthCall is an app which aims to streamline residents’ care by giving staff the tools to monitor and submit observations digitally to NHS Clinical teams when concerned about their wellbeing. This helps clinical staff to triage referrals to get residents the most appropriate care first time.

Key findings include:

  • Reduced Emergency Attendances: HealthCall contributed to an 11% reduction in emergency department attendances from care homes
  • Fewer Emergency Admissions: There was a 25% decrease in emergency admissions among care home residents
  • Minimised Readmissions: HealthCall curbed readmissions within 28 days by 29%
  • Shortened Length of Stay: The study found an 11% reduction in the length of stay for emergency admissions
  • Cost analysis reported a reduction of £57 per resident in 2018, increasing £113 per resident in 2021.
